Introduction and Acceptance of Terms

Welcome to Quick Test Health Services, a locally-owned, CLIA-certified medical testing laboratory located at 7165 E University Dr #155, Mesa, Arizona 85207. These Terms and Conditions ("Terms") govern your use of our laboratory testing services, including but not limited to drug testing, STD testing, DNA testing, blood work, TB testing, DOT physicals, N95 fit testing, and LiveScan fingerprinting services in Mesa, Apache Junction, Queen Creek, Gilbert, Chandler, Tempe, and surrounding East Valley communities.

By accessing our services, scheduling an appointment, or using our website at www.quicktestservices.com, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.

Quick Test Health Services is a locally-owned business drawing on extensive healthcare experience to provide professional, confidential, and accurate medical testing services to individuals, businesses, legal entities, and healthcare providers throughout Maricopa County, Arizona.

2. Services Offered

2.1 Medical Testing Services in Mesa, AZ

Quick Test Health Services provides testing services as a CLIA-waived certified collection facility, including:

  • Drug Testing Services Mesa: DOT and non-DOT testing (urine, hair, breath alcohol) for employment, court-ordered testing, and accident screening.

  • STD Testing Mesa: Confidential sexually transmitted disease testing (HIV, Chlamydia, Gonorrhea, etc.) without insurance or referral required.

  • DNA Testing Mesa: Paternity, maternity, sibling, grandparentage, and legal DNA testing.

  • Blood Work Mesa: Hormone panels, thyroid testing, CBC, metabolic panels, and health panels.

  • Other Services: TB Testing, DOT Physicals, N95 Fit Testing, LiveScan Fingerprinting, and Mobile Testing Services.

2.2 Testing Standards and Certification

Quick Test Health Services is a CLIA-waived certified collection facility (Clinical Laboratory Improvement Amendments) operating in full compliance with federal regulations and Centers for Medicare & Medicaid Services (CMS) standards. We partner with SAMHSA-certified laboratories for all drug testing analysis, and all results are verified by licensed Medical Review Officers (MRO) as required by Arizona drug testing laws and DOT regulations.

Arizona Drug Testing Compliance: We comply with Arizona Revised Statutes (ARS) Section 23-493.03(5), which requires drug testing to be conducted only at laboratories certified by the United States Department of Health and Human Services or the College of American Pathologists.

3. Appointments and Walk-In Services

3.1 Scheduling and Availability

Quick Test Health Services welcomes both walk-in clients and scheduled appointments at our Mesa testing facility. Most appointments take approximately 15 minutes, and many rapid screening tests provide results before you leave.

For Services Purchased Directly Through Quick Test:

  • Walk-in services are available during regular business hours

  • Most visits are completed in 15 minutes or less

  • Rapid screening results are available immediately for many tests

  • Same-day appointments available for most testing services

  • Prepayment required to schedule appointments (walk-ins pay at the time of service)

For Third-Party Collection Services:

Quick Test Health Services also serves as an authorized collection site for certain third-party testing companies and organizations. If your test was ordered through another company (employer, attorney, testing service provider, etc.):

  • Follow the scheduling instructions provided by the ordering party

  • Bring your order form, barcode, or confirmation number

  • Payment terms are determined by the third party (you may have already paid)

  • Appointment policies and cancellation terms are set by the ordering party

  • Collection procedures follow the guidelines specified by the ordering organization

3.2 Cancellation and No-Show Policy

For Services Purchased Directly Through Quick Test:

Quick Test Health Services operates on a non-refundable payment policy. All payments made for services are final, non-refundable, and automatically convert to a non-expiring service credit for the client if the original service is not rendered.

 

Prepayment and Service Credit

 

Prepayment is required for all scheduled appointments to reserve the time slot and materials if purchasing services from Quick Test Health Services. Not third-party testing services.

  • Cancellations with 24+ hours' notice: The client's payment is held as a non-expiring service credit. The client may reschedule the same service at no extra charge or apply the credit to any other Quick Test Health Services offering.

  • Same-day cancellations: The client's payment is held as a non-expiring service credit. The client may reschedule the same service once at no extra charge. Subsequent same-day cancellations will require a new prepayment.

  • No-shows (failure to appear without notice): The client's payment is forfeited unless the client calls within 24 hours of the missed appointment. In this case, the payment is converted to a non-expiring service credit, and the client must prepay again to schedule a future appointment.

  • Walk-in services: Payment due at time of service, no prepayment required

For Third-Party Collection Services:

If your test was ordered through a third party (employer, testing service, attorney, etc.), cancellation and rescheduling policies are determined by the ordering organization. Contact the party who scheduled your test to cancel or reschedule. Quick Test Health Services follows the policies established by the ordering party.

Court-Ordered and DOT Testing: Missed appointments for court-ordered testing, probation monitoring, or DOT compliance testing may be reported to the ordering authority. These appointments must be rescheduled with approval from the requesting party. Failure to appear may have legal or employment consequences.

4. Payment Terms and Pricing:

4.1 Payment Methods

Quick Test Health Services accepts the following forms of payment at our Mesa location:

  • Cash

  • Credit cards (Visa, Mastercard, American Express, Discover)

  • Debit cards

  • FSH/HSA

  • Online payment through our secure payment portal

4.2 Payment Policy

For Services Purchased Directly Through Quick Test:

Prepayment is required for scheduled appointments. Walk-in clients pay at the time of service. We do not bill insurance directly, but we can provide itemized receipts and documentation for you to submit to your insurance provider for potential reimbursement.

For Third-Party Collection Services:

If your test was ordered through a third party (employer, testing company, attorney, etc.), payment is handled according to their terms. You may have already paid the third party, or they may bill you separately. Quick Test Health Services collects the specimen and sends it to the designated laboratory as instructed by the ordering party.

A 3% processing fee will be added to all credit card transactions to cover merchant service costs.

4.3 Pricing and Transparency

All service pricing is clearly posted on our website at www.quicktestservices.com and at our Mesa testing facility. We are committed to transparent, upfront pricing with no hidden fees.

Pricing may vary based on test complexity, turnaround time requirements, and whether mobile services are requested. Final pricing will be confirmed before services are rendered.

4.4 Corporate and Volume Discounts

Quick Test Health Services offers corporate accounts and volume discounts for businesses requiring regular testing services in Mesa and the East Valley. 5. Specimen Collection and Testing Procedures

5.1 Collection Standards

All specimen collection at Quick Test Health Services is performed by trained, certified collection specialists using federally-approved protocols. We maintain strict chain of custody procedures for all forensic, legal, and DOT testing in compliance with:

  • Department of Transportation (DOT) 49 CFR Part 40 regulations

  • Substance Abuse and Mental Health Services Administration (SAMHSA) guidelines

  • Clinical Laboratory Improvement Amendments (CLIA) standards

  • Arizona drug testing laws and procedures

5.2 Direct Observation Collections

In accordance with 49 CFR Part 40.67, direct observation collections may be required when there is reason to believe the donor may attempt to adulterate, substitute, or tamper with a specimen. Direct observation will be conducted by a same-gender collection specialist in a professional manner designed to maintain donor dignity while ensuring specimen validity.

5.3 Specimen Validity Testing

Quick Test Health Services reserves the right to reject specimens that exhibit signs of adulteration, substitution, or tampering, including but not limited to:

  • Temperature readings outside the acceptable range

  • Abnormal specimen appearance or odour

  • Presence of adulterants or foreign substances

  • Specific gravity, pH, or creatinine levels outside normal parameters

Refusal to Test: Under Arizona and federal regulations, refusing to provide a specimen, attempting to adulterate a specimen, or leaving the collection site before completing the process constitutes a refusal to test and will be reported as such to the ordering party. A refusal to test is treated the same as a positive test result for employment purposes.

5.4 Privacy During Collection

Donors cannot leave the collection facility between providing the specimen and completing the chain of custody documentation. Requests to leave the facility to purchase beverages or for any other reason will be denied in accordance with DOT protocol to maintain specimen integrity and chain of custody.

6. Test Results and Reporting

6.1 Turnaround Times

Result turnaround times vary by test type. These are the general industry estimates:

  • Rapid/Instant Tests: Results available in 15 minutes (screening only).

  • Standard Lab Work/Negative Drug Tests: Typically 24-72 hours.

  • Specialized/Confirmed Positive Tests: 3-5 business days or longer.

Delays and Holidays Disclaimer: All stated turnaround times are estimates provided by the laboratory and are not guaranteed. Quick Test Health Services is not responsible for delays caused by laboratory backlogs, equipment failure, shipping/courier issues, severe weather, national holidays, or any federal/state-mandated laboratory closures, or the need for additional confirmatory testing.

6.2 Result Notification

Test results will be released according to the method specified at the time of collection:

  • Directly to the donor (for personal testing)

  • To the designated employer contact (for workplace testing)

  • To the ordering attorney or court (for legal testing)

  • To healthcare providers as directed

Results are typically communicated via secure email, phone call, or through our secure online portal. For legal or court-admissible testing, official documentation will be provided with appropriate certifications and signatures.

6.3 Confidentiality of Results

In compliance with HIPAA and Arizona Revised Statutes Section 23-493.06, test results are strictly confidential and will only be released to:

  • The tested individual (except in certain DOT and court-ordered situations)

  • The employer or designated employer representative (for workplace testing)

  • Medical Review Officers (for verification purposes)

  • Legal entities with proper authorization (courts, attorneys with valid representation)

  • Authorized medical personnel treating a medical emergency

HIPAA Compliance: Quick Test Health Services maintains full compliance with the Health Insurance Portability and Accountability Act (HIPAA) and all Arizona healthcare privacy laws. Unauthorized disclosure of test results may result in legal penalties.

6.4 Accuracy and Medical Review

All positive drug test results undergo confirmation testing using a different analytical method (typically GC/MS or LC/MS/MS) as required by Arizona law. Confirmed positive results are reviewed by a licensed Medical Review Officer (MRO) who will contact the donor to discuss legitimate medical explanations before reporting results as positive.

Quick Test Health Services partners with SAMHSA-certified laboratories and other accredited facilities to ensure the highest accuracy standards. While we take every precaution to ensure accurate results, no test is 100% infallible. Results should be interpreted within the appropriate clinical context.

7. Arizona Drug Testing Law Compliance

7.1 Employer Testing Rights

Under Arizona Revised Statutes Title 23, Chapter 2, Article 14, Arizona employers have the right to maintain drug-free workplaces through testing programs. Quick Test Health Services supports employers throughout Mesa, Gilbert, Chandler, Tempe, and the East Valley in implementing compliant workplace drug testing programs.

7.2 Written Drug Testing Policy Requirement

Arizona law requires employers to maintain a written drug testing policy that includes:

  • Statement of employer's policy on drug and alcohol use

  • Testing for the DOT 5-panel (marijuana, cocaine, amphetamines, opioids, PCP)

  • SAMHSA-certified laboratory analysis

  • Medical Review Officer (MRO) review of all results

  • Split specimen procedures

  • Direct observation when required

  • Shy bladder protocols

  • Proper chain of custody documentation

8.3 DOT Physicals and Medical Examinations

Our medical examiners are certified by the Federal Motor Carrier Safety Administration to conduct DOT physical examinations for commercial motor vehicle drivers. Medical examinations are conducted according to FMCSA guidelines and include evaluation of vision, hearing, blood pressure, urinalysis, and overall physical fitness for commercial driving duties.

8.4 FMCSA Clearinghouse Reporting

Quick Test Health Services reports all DOT drug and alcohol violations to the FMCSA Drug and Alcohol Clearinghouse as required by federal law for commercial motor vehicle drivers. Employers and drivers can access clearinghouse records through the FMCSA online system.

Quick Test Health Services provides legally-defensible, court-admissible testing for:

Direct-to-Consumer (DTC) Lab Testing and Blood Work

11.1 NO MEDICAL ADVICE

Quick Test Health Services is a CLIA-waived collection facility and is NOT a physician's office. Our staff are collection specialists, not medical providers. We do not provide medical advice, diagnosis, treatment, interpretation, or consultation for lab results. You must consult with a licensed physician for interpretation and treatment.

11.2Physician Order and Supervision (Updated)

NO DOCTOR REFERRAL NEEDED: Quick Test Health Services is a Direct-to-Consumer (DTC) collection site operating under the clinical direction of our Medical Director.

  • No Interpretation or Diagnosis: The lab test you have chosen is not a substitute for a comprehensive examination by a health care provider and should not be used to diagnose or predict an illness. Quick Test Health Services staff, the Medical Director, and the authorized ordering provider will not analyze, evaluate, critique, or otherwise interpret the results of your tests.

  • Client Responsibility: It is your sole responsibility to consult your own licensed medical doctor for interpretation, analysis, evaluation, and explanation of your test results and to take appropriate action regarding those results.

  • Ordering Provider Role: You acknowledge that a licensed medical provider's order (requisition) is required for all laboratory testing in Arizona. Quick Test Health Services fulfills this regulatory requirement by utilizing an authorized provider, such as our Nurse Practitioner (NP), who provides the necessary written orders (requisitions) for your specific requested tests. The cost of this authorized ordering service is included in the total price of your test.

11.3 Limitation of Liability

The Client agrees that Quick Test Health Services, including its officers, agents, employees, Medical Director, and Nurse Practitioner (NP), shall not be liable to the Client for any claims, actions, or damages arising from, or in any way related to:

  • Inaccurate, delayed, or misinterpreted test results.

  • Errors, failures, or delays caused by the independent testing laboratory, the Medical Review Officer (MRO), or any courier/shipping service.

  • The final medical diagnosis, treatment, or any damages resulting from the use or misuse of the test results by the client or any third party.

Quick Test Health Services acts solely as a specimen collection site and facilitator.

11.4 Third-Party Laboratory Relationships

Quick Test Health Services partners with SAMHSA-certified and CLIA-certified third-party laboratories for specimen analysis. While we carefully select laboratory partners based on quality and accreditation standards, we are not liable for laboratory processing delays, analytical errors, or other issues arising from laboratory operations beyond our direct control.

12. Client Conduct and Facility Rules

12.1 Professional Environment

Quick Test Health Services maintains a professional, respectful testing environment. All clients, donors, and visitors are expected to:

  • Treat staff and other clients with respect and courtesy

  • Refrain from disruptive, threatening, or inappropriate behaviour

  • Follow staff instructions regarding testing procedures

  • Maintain confidentiality regarding other clients' presence or testing

  • Comply with facility rules and collection protocols

12.2 Refusal of Service

Quick Test Health Services reserves the right to refuse service to any individual who:

  • Exhibits threatening, violent, or abusive behaviour

  • Appears to be under the influence of drugs or alcohol (except where testing for impairment)

  • Attempts to tamper with specimens or collection procedures

  • Refuses to provide proper identification

  • Violates facility rules or collection protocols

  • Has outstanding unpaid balances (for non-court-ordered testing)

12.3 Zero Tolerance for Tampering

Any attempt to adulterate, substitute, or tamper with specimens will result in:

  • Immediate refusal to test report to ordering party

  • Permanent ban from Quick Test Health Services facilities

  • Notification to relevant authorities (employers, courts, probation officers)

  • Potential criminal prosecution under Arizona law

13. Website Terms and Online Services

13.1 Website Use

The Quick Test Health Services website at www.quicktestservices.com provides information about our testing services, pricing, and educational resources. By using our website, you agree to:

  • Use the site only for lawful purposes

Continued use of our services following the posting of revised Terms constitutes acceptance of the modified Terms and Conditions. We encourage you to review these Terms periodically.

For significant changes affecting client rights or obligations, we may provide additional notice via email (if we have your contact information) or through prominent website notices.

14. Class Action and Arbitration Waiver

 

To the extent permitted by law, all claims must be brought in the parties' individual capacity and not as a plaintiff or class member in any purported class, collective, representative, multiple plaintiff, or similar proceeding. Any disputes that cannot be resolved informally must be resolved by binding arbitration in Maricopa County, Arizona, in accordance with the rules of the American Arbitration Association.
