Prices may change without notice; the price quoted at check-in is the price you pay. We accept cash, credit/debit cards, and FSA/HSA for eligible services. A 3% processing fee applies to credit card transactions.

A booking request submitted through our online booking form is not a confirmed appointment until you receive confirmation. We may reschedule or decline a request when needed; if we cannot honor a requested time, we will offer alternatives.

5. Accuracy of Information
We strive to keep test descriptions, sample requirements, and turnaround estimates accurate, but we do not warrant that all information on the site is complete or current. Always confirm test specifics with our team before relying on them for a time-sensitive decision.

6. Acceptable Use

12. Governing Law
These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-laws principles. Any legal action arising out of these Terms will be brought in the courts of Maricopa County, Arizona.
