1-Step TB Skin Test (PPD): how it works

A tiny dose of tuberculin is placed just beneath the skin of the forearm. It looks for an immune reaction to TB proteins — a firm raised bump at the site means your body recognizes the bacteria. You come back 48 to 72 hours after placement so a team member can measure the site, and your certificate is handed to you same-day at that return reading. 2-Step TB Skin Test: how it works, and why the wait

The 2-step is the same PPD skin test performed in two rounds. Round one is placed and read 48 to 72 hours later. Then, after a gap of 1 to 3 weeks, a second test is placed and again read 48 to 72 hours later. Your certificate is issued same-day at that second reading.

The 1 to 3 week gap exists for a real clinical reason. If a person was exposed to TB long ago, their immune response can weaken over time and fail to react to a single skin test — producing a false negative. The first test re-alerts the immune system; the second, placed weeks later, then registers the true response. This is called the booster phenomenon, and it is why hospitals, nursing homes, and long-term care employers require the 2-step TB test for new hires rather than a single screen.
